MU Tournaments

Welcome to the Player Waiver and Registration with MonkeyUp Tournaments

This registration will allow you to input all Player and Guardian information as well as sign off on a waiver.  Please make sure you have the following information prior to filling out the waiver:

• The exact name and division of your team
  EX- 3D Georgia White 2027
• Your players' jersey number
• Your players' position
• Your players' date of birth
• Your players graduation year

All players are required to fill out a players registration and waiver in order to participate in a MonkeyUP Tournament.  In addition, players and parents are required to sign a code of conduct.

High School students will have an opportunity to fill out information for college coaches.  This is not mandatory.  Information will never be sold, but we do have coaches who attend and ask for student information.  You can opt out if you wish.
